Madera Rehabilitation & Nursing Center had the largest bed capacity among nursing homes in Madera County during the first quarter of 2026, the Centers for Medicare and Medicaid Services (CMS) reported.

The facility offered 176 beds, surpassing the Madera County average by 96 beds for the first quarter of 2026. On average, it served 167 residents during that stretch.

The CMS assigned Madera Rehabilitation & Nursing Center an overall rating of 2 out of 5, lower than California’s statewide nursing home average of 3.2.

Majestic Mountain Care Center was the county’s second largest with 66 beds, while Palms Care Center ranked third with 65 beds for the quarter.

By the close of the quarter, Madera Rehabilitation & Nursing Center had paid $373,733 in three fines and accumulated five penalties.

There were four for-profit nursing facilities operating in Madera County, California, during this period.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
